چکیده
Oculomotor palsy is one of the most frequent neuro-ophthalmologic complications in patients with diabetes. Specifically, sixth nerve common. A 51-year-old female a past medical history type 1 diabetes on insulin and hypertension presented complaints worsening headache diplopia. Physical exam was remarkable for recurrent palsy. Patient hemodynamically stable. Labs showed blood glucose 367 mg/dL (70-100 mg/dL), an anion gap 2 mmol/L (3-13 mmol/L), creatinine 0.61 mg/dl (0.66-1.25 sodium 129 (135-145 potassium 4.4 (3.5-5.1 mmol/L). Her hemoglobin A1C 13.0% complete count shows white cell 6,000/uL (3,600-10,700/uL) 14 g/dL (13.0-18.0 g/dL). Magnetic resonance venography head negative cerebral venous sinus thrombosis magnetic imaging brain acute abnormalities. Blood readings improved during hospitalization after optimizing her regimen. She recommended to follow-up outpatient endocrinology discuss use pump better control Sixth-nerve but lesser-known complication poorly controlled diabetic patients. Studies have shown higher incidence sixth-nerve palsy, warranting more focused evaluation along close observation tighter levels A1C.

Isolated sixth nerve palsy
An isolated sixth nerve palsy may be a harbinger of underlying intracranial disease. Due to its long subarachnoid course, it may be damaged by downward shift of the brainstem as often occurs in increased or decreased intracranial pressure (“false-localizing sixth nerve palsy”). Alternatively, the sixth nerve may be involved in isolation by a compressive lesion in the cavernous sinus or along th...

Congenital abduction deficit is most likely due to Duane's retraction syndrome as congenital abducens nerve palsy is very rare. We report two cases of infantile abduction deficit due to sixth nerve palsy associated with other anomalies to highlight the importance of including neuroimaging in the evaluation of an infant presenting with a limitation of abduction.
متن کاملCryptococcal Meningitis Presenting with Isolated Sixth Cranial Nerve Palsy in a Patient with Systemic Lupus Erythematosus
Cryptococcal meningitis is a rare complication of systemic lupus erythematosus (SLE). The nonspecific neurologic findings associated with this infection delays accurate diagnosis because initial neuropsychiatric manifestations of SLE are in instances indistinguishable from that of crytococcal meningitis. We report a case of cryptococcal meningitis presenting with unilateral sixth cranial nerve ...
